Age UK Cheshire, in partnership with Cheshire Fire and Rescue Service (CFRS) and the Alzheimer’s Society, is establishing a Dementia Advice Service (DAS) in Cheshire East.
The service will facilitate easy access to appropriate care, support and advice for people with dementia and their carers.
Within this local DAS the Alzheimer’s Society will focus on the emotional support and Age UK Cheshire/CFRS will provide more practical based solutions.
The aim of the DAS will be to work towards addressing the current gaps in information provision about dementia across the borough and help those living with dementia to access a range of services that would improve the quality of their lives and assists them at times of need on the dementia journey from pre diagnosis to end of life care.
The Dementia Advisor post is initially funded until the end of March 2014 and is a joint post between Age UK Cheshire and the Cheshire Fire and Rescue Service (the employer will be Age UK Cheshire).
The main purpose of the role will be to establish, develop and maintain a Dementia Advice Service for older people with a diagnosis of dementia and their carers. The post holder will work alongside our existing Supporting You service.
This is a full time post (36.25 hours per week) based at the Age UK Cheshire office in Crewe with travel required across Cheshire East. This post is subject to an Enhanced Disclosure & barring Service Check. The job holder needs to be able to travel freely across Cheshire for business purposes.
